Information for your role

The details you need.
Without the back-and-forth.

GPs & treating teams

Share the clinical concern, relevant history and the client's preferences. Please include consent arrangements for communication.

Insurers & case managers

Include the claim contact and relevant treatment decisions. Specify any report or coordination request separately.

Rehabilitation providers

Clarify the rehabilitation goals, treating-team contacts and barriers affecting everyday function or work.

Solicitors

Distinguish a treatment referral from an independent assessment or legal report. We confirm service scope before accepting the request.

Prepare the next conversation

A referral that starts
with the right information.

This checklist tool helps organise your referral. It does not collect client information, submit a referral or upload documents.

Call our team to confirm the current secure document-transfer route. Please do not enter or send health information through a general enquiry.

Scope & communication

Keep the purpose clear.

Treatment is distinct from an independent assessment or expert opinion. Let us know what is being requested so that the appropriate practitioner, fee and consent process can be considered.

We discuss what information may be shared with the client and relevant parties. A referral is not blanket consent to release the full clinical record.

Useful information to have ready

  • Client consent and relevant contact arrangements.
  • Claim number, insurer and funding decisions, where available.
